At most once every two years. The clock runs from the lease commencement date or from the date of the last increase, whichever is closer - and a landlord can only use the right to increase rent five times during the term of the lease.
Two years is the minimum gap the Property Rent Law allows between increases. A landlord who raised your rent last year cannot raise it again this year, no matter what the renewal letter says.
The five-times lifetime limit is the part almost nobody knows. On a long lease, once a landlord has used all five increases, the rent is fixed for as long as that lease runs.
